Tennessee Department of Education requires that all students entering 7th grade provide proof of Tdap booster vaccination. This proof must be on a State of Tennessee Vaccination Form that will provided by your child's doctor or the Scott County Health Department. Please contact the school office, your child's doctor, or the Scott County Health Department if you have any questions.
